When recording a tv show, are the audio and video always recorded simultaneously?

I'm wondering if sometimes the actors are required to re-enact the
audio into a microphone, if a microphone could not be placed
appropriately in the scene. Or if the scene takes place in a very
noisy setting.

If so, how could they ever possibly sync their voices with video and
it be unnoticeable?
